VCS understands the importance of a smooth transition to adulthood for young people. We are passionate and committed in offering  specialised Step-Down and Transitional Services designed to support young adults aged 16-18, and adults with Learning Disabilities (LD) and Mental Health challenges. Our comprehensive programs focus on empowering individuals to transition smoothly into greater independence and embrace personal growth.

 

Our goal is to work with Professionals, Social Care and Health, and a range of partners, in developing the necessary skills, to thrive in independent living, leading to integrating as part of our community.

Wrap Around Service

WRAP AROUND SERVICE

We collaborate with Local Authorities and organisations regulated by OFSTED, DfE and CQC to provide a holistic Wrap Around Service in Health and Social Care for Young People and Adults.

We have established a comprehensive Wrap Around Service in Health and Social Care, specifically designed to support young people leaving Children's Homes and Foster care, as well as adults with Mental Health and Learning Disabilities. Our integrated approach combines accommodation, further education, transportation, and collaboration with various professionals and community partners to create a nurturing and empowering environment for the individuals we serve.


Objectives:
> To provide holistic support and care for young people and adults in transition from residential care to independent living, focusing on their mental well-being, skill development, and overall quality of life.
> To foster an inclusive and supportive community that encourages p
ersonal growth, social interaction, and vocational training opportunities for all individuals involved.

> To create an environment that promotes self-determination, independence, and empowerment, allowing our Service Users to achieve their full potential and lead fulfilling lives.

Accommodation registered with OFSTED for Young People under the age of 18.

Accommodation registered with CQC for Adults. 

Solo-placements;

5 bedded, 4 bedded & 2 bedded Homes;

Staffed 24/7 by trained professionals, ensuring a safe and nurturing environment 

Locations are strategically placed geographically.

Registered Accommodation

Further education options registered with Ofsted and the Department for Education (DfE);

Personalised learning plans tailored to each individual's interests and abilities, allowing them to continue their education and acquire essential life skills;

Offering vocational courses including, but not limited to:

Functional skills in English and Mathematics;

Apprenticeships;

Motor vehicle maintenance;

Hair & Beauty studies; 

NOCN Level 1 and 2 courses.

Community based projects & work experience.

Career mapping.

Home Schooling on a 1:1 basis.

To ensure easy access to education and activities, we provide reliable transportation options for our clients. This will enable them to attend classes, vocational training programs, and engage in various community activities, promoting social integration and independence.

Transportation Services

Working collaboratively with Psychologists, Therapists, and other Health professionals to offer comprehensive mental health support to our clients with complex needs, including:

Learning Disabilities - delivering Skills for Life outcomes;

DOLS - Deprivation of Liberty Safeguards, within the parameters of Annex A.

These partnerships will enable us to identify and address specific needs, providing individualised care and therapy services as required.
